Phyllis J. Ladka

Phyllis J. (née Smith) Ladka, 79, of Carlisle, Pa., died Jan. 16, 2015, at Carlisle Regional Medical Center. 

Born April 15, 1935, in Mineola, she was a daughter of the late Francis J. and Carolyn Amann Smith and the widow of Karol W. Ladka, who died Dec. 24, 2014.

She was a graduate of Glen Cove Nursing School. Phyllis was a licensed practical nurse with Central Suffolk Hospital (now Peconic Bay Medical Center) in Riverhead. She was a former member of St. John the Evangelist R.C. Church, also in Riverhead.

Phyllis loved to travel, and especially loved the time spent with her loving family and caring for her pet dachshunds.

Surviving are three sons, Ronald J. (Donna) of White River Junction, Vt., Robert C. (Suzanne) of Jamestown, N.C., and Richard F. (Jacqueline) of Dodge City, Kan.; one sister, Carol Lang of Long Beach, N.Y.; eight grandchildren; and one great-granddaughter. She was predeceased by a sister, Patricia Steele.

The family will receive visitors Tuesday, Jan. 27, from 9:30 to 10 a.m. at Hoffman-Roth Funeral Home & Crematory, Inc., in Carlisle, Pa. Graveside services will be held at 11 a.m., Jan. 27 at Indiantown Gap National Cemetery in Annville, Pa., the Rev. William C. Forrey officiating.
